Title: Supervisor for Bridges to Health
Key Responsibilities:
- RESPONSIBILITY: Supervise 5 to 6 Health Care Integrators in the delivery of services to B2H families.
- DUTIES:
- • Provide weekly supervision for staff that includes support, education and accountability
• Ensure that Individual Health Plans, Progress Notes and all necessary case recordings are up to date.
• Maintain statistical data on all cases
• Review Health Care Integrators’ contact sheets

Title: Bilingual (Spanish/ English) Health Care Integrator
Key Responsibilities:
- The Bridges to Health Program provides family and community support services to children in foster care and their caregivers so that these children can remain in the community and avoid placement in institutional settings. These services complement existing foster care services for children who are medically fragile, developmentally disabled, or suffering from serious emotional disturbances, and can involve multiple family networks and follow the participant after the foster care case has closed.
- DUTIES:
• Engage children and caregivers to assess relevant child and family dynamics, including strengths and needs.
• Evaluate the need for Bridges to Health services; complete eligibility assessment.
• Develop and implement with child and family an integrated service plan (also known as the “Individualized Health Plan”) that outlines services needed to support the child in his or her home and community;
• Evaluate the plan’s effectiveness with child and family and update on an ongoing basis.
• Link child and family to needed supports and services, including skill building, intensive in-home supports, employment-related services, respite, crisis intervention and avoidance, advocacy, and adaptive and assistive equipment, among others; coordinate with these services.

Minimum Requirements:
- A master’s degree in social work, psychology, or other related field, plus knowledge of providing service coordination and information, linkages, and referrals for community-based services to children with special needs or individuals with disabilities. A bachelor’s degree and two (2) years of related work experience may be substituted for a master’s degree. Bilingual (Spanish-English) required.

Title: Bilingual Social Worker (Preventive Services)
Key Responsibilities:
- Responsibilities:
- 1. Provides casework and clinical services to children and families utilizing individual, group and family modalities. Coordinates services for a caseload of 10 families as recommended by NAC’s multi-disciplinary treatment team.
2. Provides intensive social work services to children and families in order to implement the goals and tasks of the Family Assessment and Service Plan (FASP).
3. Conducts home visits to families.
4. Ensures adherence to agency policies and ACS regulatory requirements.
5. Makes adequate use of individual and group supervision, on-site clinical trainings and mental health in-service.
6. Completes ACS and NAC paperwork in a timely manner and ensures that documentation conforms to agency, City and State regulatory requirements.
7. Participates as a treatment team member in agency case review meetings.
8. Has responsibility for a 24-hour on-call duty on a rotating basis.
Minimum Requirements:
- 1. MSW from an accredited school of social work; LMSW preferred.
2. Proficiency in Spanish is required.
3. Knowledge of health-related factors in working with chronically ill children, as well as experience in the area of child development is a plus.
4. Prior experience in preventive services, child welfare, and/or health care preferred.

Title: Case Aide (Per Diem)
Key Responsibilities:
- Supervises and maintains the safety of individuals on vans and in NAC playrooms. Collaborates with Activities Coordinator and other team members to arrange and implement recreation, trips and special events for NAC children and families. Supports clinical staff in the provision of consistently high quality services.
- Key Responsibilities:
Supervises medically fragile children and their siblings in agency vans from their homes to NAC and on recreational trips.

Title: Early Childhood Coordinator
Key Responsibilities:
- The Early Childhood Coordinator is responsible for:
- 1. Overall responsibility for assisting NAC families to obtain appropriate educational services for their children from birth through kindergarten age, including Early Intervention (EI) system, Preschool Special Education Services (CPSE), day care and Head Start Services.
2. Coordinates screenings, evaluations, placement and assessment of services to eligible NAC children.
3. Assists NAC families with transition from Preschool programs to appropriate kindergarten programs/services.
4. Serve as consultant and resource to all NAC staff regarding Early Childhood Development, parent/child interaction and accessing of services for young children.

Title: F/T Driver
Key Responsibilities:
- Drives medically fragile children and their families in agency vans from their homes to NAC, and on recreational trips in a manner which is safe and efficient.

Maintains vans in good working condition and conduct regular inspections, and seek repairs as needed. Ensure that the van is cleaned regularly, including seat belts, and make sure that vans have carseats. Make sure that the van has a well-stocked first aid kit and a working fire extinguisher.

Coordinates with dispatchers and case aides to organize transportation of medically fragile children and their families.

Has knowledge of the geography of all five boroughs of New York City and chooses most efficient and safest routes when transporting children.
Minimum Requirements:
- Experience driving a van (or bus or other vehicle) and working with children, or other clients with disabilities or medical conditions, etc.

Title: Fee-for-Service Psychotherapist (part-time)
Key Responsibilities:
- RESPONSIBILITY: Delivers mental health treatment services including individual, collateral, family, and/or group psychotherapy, intake and assessment, crisis intervention, and case management in New Alternatives for Children’s (NAC’s) Article 31 Mental Health Clinic Program (West 26th Street location). Completes required documentation within prescribed timeframes.
- DUTIES:
• Provides psychotherapy sessions to clients in NAC’s Mental Health Clinic;
• Documents sessions by means of progress notes;
• Assists in planning, development, and implementation of care delivered;
• Develops treatment plans and quarterly treatment plan reviews;
• Completes required documentation including, but not limited to, contact forms, progress notes, treatment plan reviews, anecdotal notations and other documentation as needed within prescribed timeframes;

Title: Quality Assurance Specialist
Key Responsibilities:
- RESPONSIBILITY: Monitoring and implementing a strategic quality improvement plan for the Bridges to Health (B2H) Waiver Program administered by New Alternatives for Children (NAC). B2H is a Medicaid Waiver Program operated by the New York State Office of Children & Family Services (OCFS) that works with children in foster care who have developmental delays, are seriously emotionally disturbed, or are medically fragile.
- DUTIES:
• Monitoring all programs and services according to agency, state, and federal funding standards and requirements.
• Conducting periodic random case record reviews and other internal quality reviews of the B2H program with written reports of findings to the Director of QA and B2H staff.
• Implementing and conducting analysis of the B2H Consumer Satisfaction Survey with written report of findings to the Director of QA and B2H.
• Monitoring agency compliance with the implementation of B2H regulations and standards.
• Attending internal and external meetings and trainings related to B2H quality assurance.
• Conducting special projects and tasks as assigned by the Director of QA.
• Providing QA for other programs and projects deemed necessary by the Director of QA

Title: RN - Pediatric Nurse/Clinical Nurse Specialist
Key Responsibilities:
- Duties:
- 1. Assess the medical needs of each child by reviewing their diagnosis, birth history, and inpatient and outpatient records.
2. Initiate, maintain, and update health care plans for each client.
3. Train foster and prevention families to care for their child in a home setting with reinforcement as needed.
4. Make appropriate referrals for community-based care and provide follow-up as needed.

Title: Social Work Supervisor
Key Responsibilities:
- 1. Reports to the Assistant Director/Director of Foster Care and Adoption, and supervises four (4) MSW level foster care and adoption social workers.
2. Ensures that social workers are conducting permanency planning activities in an appropriate and timely manner that workers are identifying the needs of each case, and are either providing directly or are accessing the entire range of clinical and concrete services that the case requires.
3. Assists supervisees in prioritizing workloads.
4. Ensures that social workers are meeting all ACS and NAC mandates and paperwork requirements.
5. Attends Family Team Conferences, Service Plan Reviews, and Case Reviews.
6. Prepares social workers for court proceedings, and be willing to testify.
7. Works collaboratively with colleagues in an interdisciplinary team model.
8. Ensures supervisees’ adherence to all agency personnel policies and procedures.
9. Has responsibility on a rotating basis for 24 hour on-call duty and Saturday office administrative coverage.

Key Responsibilities:
- Performs permanency planning for all foster children and ensures that children achieve their permanency goals in as expeditious a manner as possible.
- Assesses the needs of foster children and ensures that the appropriate range of clinical and concrete services are accessed as needed for each child.
- Assesses foster parents for their potential suitability as adoptive parents; assist foster parents in managing the children placed in their homes; and assists foster parents in their efforts to work with birth families.

Completes ACS and NAC paperwork in a timely manner and ensures that all documentation conforms to agency, city, and state regulatory requirements.
